The best way, for me, to understand how I should fill the order maintain is put a break point inside and try to do what you want to acomplish via program manually, like if you are the user. But I'm sure that you already tried this 
I don't see the in your code the CRM_ORDER_SAVE and the commit after the CRM_ORDER_MAINTAIN
